Short Term Care
- Skilled Nursing & Rehab - A rapid recovery option for patients requiring post hospital rehab and medical services due to acute illness, surgery or injury. Offers enhanced clinical services, with higher clinical staffing, all in a designated unit.
- Orthopedic Rehab - A Short Stay option for patients requiring orthopedic rehabilitation after hospital discharge but before going home. Offers enhanced post-acute clinical excellence in a designated unit.
- Cardiac Specialized Care Management – (At Monterey Rehabilitation Center, Skilled Nursing & Memory Care and Whetstone Care Center) Short term post-hospital rehab and recovery for patients with a variety of cardiac diagnoses. Services include specialized diets, rehabilitation and education to continue a heart healthy lifestyle.
- Respite care – A break for primary caregivers. This service is short-term care for individuals who need assistance with daily living activities, such as bathing, dressing and grooming as well as those who require 24-hour nursing care. Guests can stay for a few days to a few weeks. Caregivers can take a break from caregiving and rest assured that their loved one will be cared for by compassionate qualified professionals. This service can be provided on a scheduled ongoing basis or as a one time service.
Long Term Care
- Assisted Living - Combines the security of care with the comforts of home. Assisted living services are for those needing minimal assistance with daily living activities like bathing, dressing, grooming and medication management. The resident has their own private suite with the security of knowing help is there, if needed.
- Intermediate Level of Care: Separate care communities for patients needing a greater level of assistance on an ongoing basis. Our centers provide person-centered care giving decision-making directly to our residents and their support partners. The integration of all aspects of daily life allows residents to truly thrive and grow as individuals and maximizes their level of independence.
- Secure Alzheimer's Care - (At Monterey Rehabilitation Center, Skilled Nursing & Memory Care only) Specialized Alzheimer's / Dementia Care with a person-centered, relationship-based approach to care. Offers a safe, secure, home-like environment with consistent staff to promote relationships and a stable family atmosphere.
Community Services
- Amity Home Health Rehabilitation – In-home, post-hospital rehab and recovery services performed by registered nurses, licensed practical nurses, licensed therapists (Physical, Occupational and Speech), Medical Social Workers and Home Health Aides who work diligently to ensure the patient receives the assistance they need to remain in their home.
- Amity Heart Care: Provides in-home post-hospital rehabilitation focused on patients with chronic cardiac conditions or those recovering from a cardiac event or surgery with the goal of maximizing the highest level of "effective living." This is achieved by maximizing cardiopulmonary function through physical rehab, promoting self-care and compassionately addressing lifestyle modifications.
- Amity Home Health Private Duty Care – In home assistance with daily activities such as meal preparation, light housekeeping, running errands, dressing/bathing or even medication reminders.
